ProCare Dental Livingston is required by law to maintain the privacy of your protected health information (PHI), provide you with this notice of our legal duties and privacy practices, and follow the terms of this notice.

We may use and disclose your information to bill and collect payment from insurance companies or other responsible parties.

Healthcare Operations:

We may use your information to operate our practice, improve care quality, train staff, and conduct internal audits.

Other Permitted Uses and Disclosures:

We may disclose information as required by law, for public health activities, health oversight, judicial proceedings, law enforcement, or to prevent serious threats to health or safety.

You have the right to inspect and obtain copies of your health records, request amendments, request restrictions, request confidential communications, receive an accounting of disclosures, and receive a paper copy of this notice.

If you believe your privacy rights have been violated, you may file a complaint with ProCare Dental Livingston or with the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.
