Sucking is a natural, healthy reflex for infants — it's soothing, and in the earliest years it doesn't pose any real risk to dental development. The concern isn't the habit itself so much as how long it continues once permanent teeth are on the way.
Why it's completely fine at first
Most children naturally stop thumb-sucking or using a pacifier between ages two and four, well before it has any lasting effect on tooth position. Baby teeth are more forgiving of this pressure than permanent teeth will be, and most early sucking habits resolve on their own without intervention.
When it starts to matter
The concern grows once a habit continues past age four, and especially once permanent front teeth begin erupting around age six. Constant pressure from sucking can push front teeth forward or prevent the upper and lower teeth from meeting properly, a pattern called an open bite — the same way steady pressure on anything soft and still developing will gradually reshape it.
Signs the habit may already be affecting teeth
- Front teeth that visibly protrude or don't meet when the mouth is closed
- A noticeable gap between upper and lower teeth even when biting down
- Changes to the roof of the mouth's shape noticeable during a dental exam
- Speech patterns affected by tongue placement, such as a lisp
Gentle ways to help a child stop
- Praise and small rewards for hours or days without the habit, rather than punishment for lapses
- Identify and address the trigger — many children suck their thumb specifically at bedtime or when anxious
- A bitter-tasting, safe nail polish made for this purpose, used as a gentle reminder rather than a punishment
- For pacifiers, a gradual approach (fewer hours per day, then daytime-only, then just at sleep) tends to work better than an abrupt stop
